Septic Application Forms

During the septic permitting process, there are various forms and paperwork to be completed. Some can only be completed by licensed professionals, and others will need to be completed by the homeowner. To understand more about our permitting process, visit the septic permit process steps page.

Forms that can only be submitted by a licensed septic designer or professional engineer:

All of these forms except the subdivision review application can be submitted through our online portal. You will need to log in to see the forms in the portal (if you don't yet have an account, click "I want to register" on the login screen to create one). Once you are logged in, click the On Site-Sewage tab in the left hand menu to view the online forms.

Forms that can be submitted by a homeowner:

Declaration of Covenant: This document link is an example template of a declaration of covenant. A notarized and recorded declaration of covenant is required for all system types except for gravity and pressure distribution systems, along with a signed monitoring and maintenance agreement with a service provider.  For more information about this requirement, see Step 2 of Phase II: Permitting & Installation on the permit steps page. The declaration of covenant may be emailed to SHD-EHQ@snoco.org, or it can be mailed or delivered in-person to the Environmental Health front counter:
