Septic Application Forms

During the septic permitting process, there are various forms and paperwork to be completed. Some can only be completed by licensed professionals, and others will need to be completed by the homeowner. To understand more about our permitting process, visit the septic permit process steps page.

Declaration of Covenant : The form here is an example template of a declaration of covenant. A notarized and recorded declaration of covenant is required for all system types except for gravity and pressure distribution systems, along with a signed monitoring and maintenance agreement with a service provider.  For more information about this requirement, see Phase II: Permitting & Installation, Step 2 under our permit process page.
