Did you know that you can access the Washington State Immunization Information System (WAIIS) without having a medical provider sign off? Now you can!
You can now access the Schools and Child Care Immunization Module and get access to current CIS forms all on your own. Accessing the module, you may even be able to eliminate the need to submit the Immunization report to WADOH at the end of the year. Click the link to get more information.
The process takes about 30 days to complete.
Once you have been granted access, follow the instructions on step 2 in order to set up the module for your program: Washington State Immunization Information System (IIS) School and Child Care Immunization Module | Washington State Department of Health
