Certified Death Certificates
The Vital Statistics office issues certified copies of death certificates for deaths occurring in the city of Everett from 1953 to the present and
for deaths occurring in the rest of Snohomish County from 1960 to the present. The fee is $20.00 per certified copy.
The Center for Health Statistics
issues death certificates for all deaths occurring in the State of Washington from 1907 to the present.
What information do I need to provide?
The following information is required:
Name of the deceased ( First, Middle, Last )
Date of Death ( or 10 year period to search )
Place of Death ( City )
How do I order a death certificate?
There are four ways to order a death certificate:
Mail Order
When ordering by mail, regular processing time is mail time each way plus two business days. Death certificate fees are
$20.00 per copy. Make checks/ money orders payable to Vital Statistics.

To use a credit card online, visit the Vitalchek website
at www.vitalchek.com.
All credit card orders receive expedited handling. You can expect your certificate in approximately 2-3 business days.
What to expect on your credit card bill using VitalChek services:
The fee for birth and death records is $20.00 each. There is an additional fee of $13.50 for ordering by credit card. Certificates are mailed out the next working day.
Order by Fax
Credit card orders by fax. You may fax your order to (425) 339-5218. Your fax request cannot be processed unless we have all the
information requested below. Death certificate fees are $20.00 per copy. There is an additional fee of $13.50 for ordering by credit
card. Certificates are mailed out regular mail the next working day. Federal Express delivery is available at an additional cost.
Address certificate is to be sent to. If you are requesting Federal Express
delivery (for an additional fee) we must have a street address. Federal
Express cannot deliver to a post office box or rural route number.

In Person
You may come to Snohomish Health District/Health Statistics and Assessment office in person at 3020 Rucker Ave,
Room 102 and fill out a short form, pay the required fee, and pick up a certified copy immediately. We are
open Monday through Friday, and issue certificates from 8 AM to 4:30 PM. Death certificate fees are $20.00 per
copy. Cash and checks are accepted, as well as a debit card for an additional fee. (Please note: VISA debit cards using the Interlink network will not work in our system.)
How do I get corrections made on a death certificate?
