Certified Birth Certificates
The Vital Statistics Office issues certified copies of birth certificates for anyone born in the State of Washington from 1926 to the present. Prior to 1926, the Vital Statistics office can only issue birth certificates for births that occurred in Snohomish County and these records are not complete. Please contact our office before requesting a Snohomish County birth prior to 1926, and we will verify that it is on file. The fee is $20.00 per certified copy.

The Center for Health Statistics issues birth certificates for all births occurring in the State of Washington from 1907 to the present.

How much does a birth certificate cost?
What information do I need to provide?
The following information is required:
  • Name of Child
  • Child's Birthdate
  • Place of Birth
  • Mother's Maiden Name
  • Father's Full Name

How do I order a birth certificate?
There are four ways to order a birth certificate:
  • Mail Order
    When ordering by mail, regular processing time is mail time each way plus two business days. The birth certificate fee is $20.00 US per copy. Make checks/ money orders payable to Vital Statistics. The mailing address is:

  • Ordering Online
    To use a credit card online, visit the Vitalchek website at www.vitalchek.com. All credit card orders receive expedited handling. You can expect your certificate in approximately 2-3 business days.

    What to expect on your credit card bill using VitalChek services:

    The fee for birth and death records is $20.00 each. There is an additional fee of $11.00 for ordering by credit card (when ordering for more than one person, the fee on each additional person is $5.00). Certificates are mailed out the next working day.

  • Order by Fax
    Credit card orders by fax. You may fax your order to (425) 339-5218. Your fax request cannot be processed unless we have all the information requested below. The birth certificate fees are $20.00 per copy. There is an additional fee of $11.00 for ordering by credit card. Certificates are mailed out by regular mail the next working day. Federal Express delivery is available at an additional cost of $17.50 to most locations.

    1. Type of credit card you are using.
    2. Credit card number
    3. Credit card expiration date.
    4. Person’s name on the credit card.
    5. Address certificate is to be sent to. If you are requesting Federal Express delivery (for an additional fee) we must have a street address.  Federal Express cannot deliver to a post office box or rural route number.
    6. Name at Birth
    7. Date of Birth (m/d/y)
    8. City or county of birth
    9. Father’s full name
    10. Mother’s full maiden name
    11. Your current address
    12. Day time phone number with area code.


  • In Person
    You may come to Snohomish Health District/Health Statistics and Assessment office in person at 3020 Rucker Ave, Room 102 and fill out a short form, pay the required fee, and pick up a certified copy immediately. We are open Monday through Friday, and issue certificates from 8 AM to 4:30 PM. The birth certificate fee is $20.00 per copy. Cash and checks are accepted, as well as a debit card for an additional fee.

  • Please arrive no later than 4:00 PM to start the process.
