that some of these illnesses have become very rare. But many diseases still exist and immunizations are very important for keeping
children healthy. Child care providers are responsible for keeping track of children's immunization status. Use the Keep on Track

Parents or legal guardians have the right to choose not to immunize their child due to medical, religious or philosophical reasons.
Parents or legal guardians must sign the appropriate box on the back of the Certificate of Immunization Status form to exempt their
child from receiving required vaccines. There are risks when choosing against vaccination, such as becoming infected with a vaccine-preventable
disease if exposed to the illness. A child who is not fully immunized also may be excluded from attending school or child care during an outbreak
of a vaccine-preventable disease. The documents below are downloadable and can be customized for your use;
