Oral Health Program > Materials - Loan library for educators

Reserving Materials from the Dental Loan Library
Materials are available on a first come-first served basis for health care professionals and teachers in Snohomish County.

Instructions for using the Oral Health Loan Library.
Please request items at least two weeks in advance of the pick up date. Return materials within one week after your use. Use the check-out form to request materials you would like to borrow. This form can be

faxed in your request 425.339.8726;
print out a copy and mail in.

Pick up the materials you have requested from the receptionist at either the Everett or Lynnwood Snohomish Health District office.

Tooth Tutor
Curriculum guidelines (1997) for Kindergarten to Sixth grade approved by the Office of the Superintendent of Public Instruction. 20-30 minute developmentally appropriate lesson plans.
Tooth Tote
Suitcase of visual aids to conduct all the lesson plans in the Tooth Tutor. Includes: models and toothbrush, Storybook-"How Many Teeth", Poster-Parts of the Tooth, Poster-Food Pyramid, Video-Seal in a Smile, Video - Spit Tobacco.(K-6th grade)

Reserve your materials by email or fax by using our check-out form.
Please use our check-out form to request materials you would like to borrow. This form can be sent to us by fax, email, or print out a copy and mail in. Pick up the materials from the receptionist at either the Everett or Lynnwood Snohomish Health District office. Please make your request at least two weeks in advance of the pick up date. Return materials within one week after your use. As time and funding are available, materials can be mailed or an alternative pick up site may be accommodated.
