When you are part of the Maternal Child Health Home Visiting Program
A Public Health Nurse will visit with you and your family to learn about your interests and concerns related
to health and family.
The Public Health Nurse can introduce you to other maternal child health team members (Community Health
Outreach Worker, Social Workers, and Nutritionist) who can also offer assistance with issues specific
to you and your family.
We will provide information about health and explore with you how to make healthy choices for
yourself and your family.
We will tell you about what is available in your community and assist you in connecting
with new resources.
We will assess individual and family needs and talk with you about
Pregnancy
Changes during pregnancy
Preparing for labor and delivery
Post partum care
Preparing for the baby
Infants Children including, children with special health care needs
