The Community Health Division
  • promotes healthy living, prevents disease and injury, and helps people get the services they need.
  • provides education, services, and referrals to individuals, families, populations at risk, and communities within Snohomish County.
  • collaborates with individuals, providers, policy makers, and organizations to develop and maintain strategies for addressing public health issues.


The Community Health Division works to achieve:

Nurse Family Partnership for Young First Time Mothers
The Nurse Family Partnership Program offers home visits to first time mothers, 21 years of age and under. The goals of the program are to improve pregnancy outcomes, improve child health and development and improved maternal like course development.
 
Healthy and Intended Pregnancies
The Unintended Pregnancy Prevention Program provides on-site nursing visits at the Sky Valley and Smokey Point DSHS Community Service Offices.

The First Steps Program offers the skilled services of a Public Health Nurse, Nutritionist, and Social Worker, also called a Behavioral Health Specialist, during and following your pregnancy. This program targets maternal and child health problems that emerge early in the life cycle and that may be prevented with improvements in maternal health habits, parental behavior, and the psychosocial and material context in which the family is functioning. The family can be seen until the baby is a year old. These services may be provided in a clinical or home setting.
 
Health and Safety in Child Care and Preschool
To safeguard children in their care, child care providers must have accurate and timely information on health and safety issues. Child Care Health Program has developed an overall strategy for providing health and safety services for child care providers in Snohomish County. This comprehensive plan of service includes child care facility assessments, consultation, training, communicable disease investigation and referral to community resources. Child Care Health Program has been instrumental in helping to create child care environments that are safe, healthy and nurturing.
 
Injury Prevention
The Injury Prevention Program is committed to reducing the incidence of both death and disability from unintentional injury among the youth of Snohomish County. We work primarily to promote bicycle safety and motor vehicle safety, though water safety and gun storage are also important aspects of our work.
 
Kids Get Care Guide to Health Care
In Snohomish County a growing number of families with limited income are struggling to find affordable health care. This guide concentrates on access to health care for children in the county. It provides information about the county’s public resources for health care. Available in English, Spanish and Russian.
 
Optimal Health for Children and Families
The Parent and Child Health Program addresses access to health care and social services, promotion of healthy lifestyles and prevention of child abuse and neglect through public health nursing visits to families referred to SHD

In the Children with Special Health Care Needs Program, a Public Health Nurse helps families to navigate the health care system and to understand special health needs of their child.
 
Oral Health
The Oral Health Program works to reduce existing oral health disparities by providing school based sealants and linkage of children with Medicaid to dentists. Activities also include community oral health assessment (SMILE Survey) and education of health care providers and educators.
 
Prevention of vaccine preventable diseases
Immunizations are provided for children and adults at SHD clinics and in the community. Travel immunizations and prescriptions are provided for individuals based on country specific itineraries.
 
Reducing the Risk and Impact of Child abuse
The Early Intervention Program provides Public Health Nurse visits to families at risk for child abuse or neglect.

In the Foster Care Passport Program, a Public Health Nurse summarizes health records and recommends health care needed by children in foster care.
 
Tobacco Prevention and Control
The Tobacco Prevention and Control Program provides information, education, resources and technical support to Snohomish County communities and partners in order to build their capacity to advocate for social change regarding the use of tobacco and exposure to tobacco smoke.
 
Wellness & Community Mobilization
Supporting communities to increase access to physical activity and improve nutrition in order to reduce obesity-related chronic disease.
