Clinic services include:
  • Emergency Contraception Medication

  • Titers to confirm immune status: measles, mumps, rubella, varicella, Hep A,and B, rabies.

  • Travel prescriptions
  • Tuberculosis testing and referral for treatment
Payment
  • Payment due on the day of service by check or cash. No credit or debit cards.
  • SHD does not bill private insurance.
  • Most medical coupons are accepted.
  • Medicare is accepted for flu and pneumonia, non-HMO client.
Fees
  • Office visit fee <21 $26.00 > 21 $14.00
  • Most services are offered on a sliding fee scale based on household size and gross combined household income. 10% of total fee will be charged.
  • Services solely for travel do not qualify for sliding fee
  • Travel consult fee $50.00 per person
  • Travel prescription fee 25.00 per person
  • Lab fees vary
Cost of vaccines:
  • Clients < 19 years of age, receiving Vaccine for Children program (VFC) vaccine are charged an administration fee of $15.00 per vaccine
  • Clients receiving non VFC vaccine are charged and $20.00 administration fee plus the price of each vaccine. Vaccine prices vary.
Under 18
  • Clients under 18 must be accompanied by a parent or legal guardian.
  • A complete record of all immunizations to date is required for all children through high school and is preferred for anyone receiving immunizations.
