Nurse-Family Partnership (NFP) is an evidenced based home visitation program available for first time mothers, 21
years of age and under. Specially trained Public Health Nurses visit families in their home throughout the
pregnancy and continue to visit until the child is 2 years old. The nurses visit weekly and bi-weekly during
the pregnancy, infancy and toddler period. The education curriculum includes information during each phase of
the development. This relationship based model builds on the parent’s strengths supporting the parent child
relationship, healthy parent-child outcomes and goals for the future helping first time parents succeed.
A public Health nurse will help you:
- Have a healthy pregnancy and a healthy baby
- Become a better parent
- Build a strong network of support for you and your baby
- Make your home a safe place for your baby to live and play
- Get referrals for healthcare, childcare, job training and other support services available in Snohomish County
- Find ways to continue your education and develop job skills
- Set goals for your family's future and find ways to help you reach them
Who can enroll in the program?
Any woman who is:
- Pregnant with first child, single or married, mothers 21 years of age and under.
- Meets income requirements.
Women can join as early in pregnancy as desired, but must join before the 24th week of pregnancy.

This program is free to all eligible women!
