Death Certificates
The Vital Statistics Office of Snohomish Health District issues certified copies of death certificates for deaths occurring in the city of Everett from 1953 to the present and for deaths occurring in the rest of Snohomish County from 1960 to the present.
The WA DOH Center for Health Statistics issues death certificates for all deaths occurring in the State of Washington from 1907 to the present.
How much does a death certificate cost?
What information do I need to provide?
  • Name of the deceased ( First, Middle, Last )
  • Date of Death ( or 10 year period to search )
  • Place of Death ( City )
How do I order a death certificate?
There are four ways to order a death certificate:
Order By Mail
Complete the Death Certificate Order By Mail Form and mail it with checks/money orders payable to Vital Statistics. The death certificate fee is $20.00 US per copy. The mailing address is:

The regular processing time is mail time each way plus two business days.
Order On-line
To use a credit card* online, visit the Vitalchek website at www.vitalchek.com. All credit card orders receive expedited handling. You can expect your certificate in approximately 2-3 business days.
What to expect on your credit card bill using VitalChek services:
  • The fee for death certificate is $20.00 each.
  • Credit card processing fee is $13.50.
  • UPS delivery is available at an additional cost.
Order By Fax
Credit card* orders by fax. You may fax your order to 425.339.5218. Use the Death Certificate Order By Fax Form to submit your order. Your fax request cannot be processed unless we have all the information requested below:
  1. Type of credit card you are using
  2. Credit card number
  3. Credit card expiration date
  4. Person’s name on the credit card
  5. Address certificate is to be sent to ( UPS cannot deliver to a post office box or rural route number, you must include a street address. )
  6. Name of the deceased
  7. Date of death ( 10 years period to search )
  8. City place of death
  9. Your current address
  10. Day time phone number with area code
What to expect on your credit card bill:

  • Credit card processing fee is $13.50.
  • UPS delivery is available at an additional cost to most locations.
Order In Person
You may come to Snohomish Health District/Health Statistics and Assessment office in person at 3020 Rucker Ave, Room 102 and fill out a short form, pay the required fee, and pick up a certified copy immediately. We are open Monday through Friday, and issue certificates from 8 AM to 4:30 PM. The birth certificate fee is $20.00 per copy. Cash and checks are accepted, as well as credit or debit card.
You may complete the Death Certificate Form before visiting our office.

How do I get corrections made on a death certificate?
  • Please contact the office to learn about the process or to learn what documents are needed. See "In Person" details above for office contact information.
  • Please arrive no later than 4:00 PM to start the process.
